Future Hope Behavioral Services

Future-Hope Behavioral Health Services, Inc Psychiatric Rehabilitation Program and Outpatient Mental Health Center (OMHC) mission is geared in expanding the community integration of our members and enable them to normalize, successful and satisfied in the environment of their choice by working towards developing useful rehabilitation goals together. The program assists the clients in simplifying their preferences for onward independency. 


As our services are purely tailored and designed for individuals with Severe and Persistent Mental Illness (SPMI) in order for them to develop or restore independent living and social skills, including the ability to make decisions on their own regarding: life, self-care, illness management, and community participation; we also promote access to and use community resources to facilitate the individual’s integration into the community, thereby expediting recovery, and inhibiting relapse and re-hospitalization. Services may be provided in our on-site facility, or in a setting most beneficial to promoting the individual participation in community life. Future Hope PRP services include PRP basic case management functions, such as assistance in securing entitlements, coordination of services, and liaison with external services (somatic and mental health) within the provision of PRP services. We provide a wide and unique varieties of supportive services using skills teaching model for persons with serious and persistent mental illness. Classes are provided in industrial arts, domestic arts, such as proper nutrition, shopping, dressing, health and medication management, communications skills, leisure and community relations, and job preparation. The program does provide recreational and social opportunities for clients who have a serious and persistent mental illness and for those who meet the criteria. Other wide-ranging focus of these choices are engrossed in the following:

A.   Every individual in the program has a Rehabilitation Counselor/Specialist assigned to him/her. The specialist counselor works directly with the client to develop an Individualized Rehabilitation Plan (IRP). A detailed IRP provides a plan which includes the clients’ long-term objectives and short-term goals. These objectives and goals need to be completed to achieve those objectives. Assessments are completed every six months to determine the areas of critical skill shortfalls. These shortfalls are further addressed through skill programming and skill teaching models to overcome the difficulties presented. 

Future Hope Behavioral Services

The leadership of FHBHS is the administrative part of the organization. It comprises of the Chief Executive Office, Vice president, Medical Director, Financial Officer and Secretary of the organization. Each officer has responsibilities towards the organization and in making the organization succeed. We have 35 combined years in both healthcare and mental health. 


We are here to offer respectful but individualized person-centered services in order to meet consumer needs.   We provide services in the community, home to individuals, or services at any other appropriate setting conducive for consumers. We will provide Baltimore County and environments with a Psychiatric Rehabilitation Program (PRP) that sustains high quality, that embedded in community  integration within mental health service. We believe in and provide community outreach programs and sponsor programs that create Mental Health Awareness in the community. 


PSYCHIATRIC REHABILITATION PROGRAM (PRP) TREATMENT MODEL

The goal and treatment model of Future Hope Behavioral Health Services, Inc are directed toward reduction of emotional or behavioral problems, providing individuals to improve and restore independent living and social skills that enhances the support for their recovery and ability to make an inform decisions and choices within the community of their choice. 


• To provide individualized care to people with Mental Disorders.


• To help individuals with behavioral health issues recover and return back from diminishing conditions to perform fully as independent and able-bodied individuals in the community.


• To help individuals improve and restore independent living and social skills needed to support their recovery and capability in making informed decisions and choices within the community of their choice.



However, the restoration of age-appropriate skills in PRP include the following: 


·      Facilitating and enhancement of consumer’s independent living and social skills; this includes consumer’s ability to make his/her decisions about his or her life regarding his or her choices of home, school or employment or community.




·      Promoting available community resources to integrate the consumer into the community.


·      Rehabilitative activities (such as individual sessions, psychoeducational groups, family groups) which will be incorporated in the PRP are as follows: 


-Self-care skills (i.e., hygiene, grooming, dietary planning, food preparation, age-appropriate involvement with self-administration of medication.


-Social skill (i.e., conflict resolution training, improved peer interactions, anger management, improved interaction with authority figures, assertiveness training, assessing risks in social environments).



-Independent living skills (i.e., maintaining personal living space, community awareness, mobility skills, monetary management, housekeeping, the importance of being punctual in all appointments, task analyses, completion of chores, staying safe while in the community, education on age-appropriate meal preparation, doing laundry.



-Leisure and recreational activities (i.e., developing of healthy leisure activities, use of leisure to develop teamwork and peer support behaviors, promotion of productive and rewarding use of time spent alone).



-Schedule planning (i.e., daily schedule planning, time management, development of daily routine, development of routine before going to bed at night to prepare for the next day).



-Cultural interest (i.e., exploration of activities that support the youth’s cultural interests by self-expression through artistic modalities.



-Age-appropriate techniques and resources (i.e., to provide understanding of the role and effects of medication and symptom management).



-Age-appropriate techniques and resources (i.e., to understand basic healthy living, nutrition, exercise, sexuality, and health education risk reduction. 

PYSCHIATRIC REHABILITATION PROGRAM (PRP)

FHBHS Objective & Services 

A Wraparound Support

Out-Patient Mental Health Center

Our Outpatient Mental Health Center provides mental health assessments, individual and group therapy, and medication monitoring.


• Within the scope of our Outpatient Clinic, we would also serve individuals with developmental disabilities who have been dually diagnosed with co-occurring mental health issues are also served.


• We render Medication Management/Psychiatric Evaluation


• Individual Therapy (one-on-one individual sessions with a therapist)


• Family Therapy — designed to provide and address specific issues that will affect the psychological health of the family, such as major life transitions or mental health conditions.


• Group Therapy (a generalized dynamic therapy with strong individuals’ dynamism) 


Psychiatric Rehabilitation Program (Adult & Children)

• Future Hope's Outpatient Substance Abuse Treatment is a 28-week treatment modality that incorporates both group and individual sessions, 1 time a week (90-120 min) onsite at our office to help individuals with substance abuse education, prevention, intervention, relapse maintenance, emotional management, and recovery management. This is done on Saturdays 11am-1PM.

Intensive Out-Patient Services

• This is a 28-week treatment modality that integrates onsite at our office to aide individuals with substance abuse education, prevention, intervention, relapse maintenance, emotional management, and recovery. This is once (1) a week, Saturdays (90-120 minutes) onsite at our office, 9am-11am.

Objectives

Our staff members strive high in their work and commit to our clients and the type of work we do on daily basis to fulfill our commitment.

• Our staff directly works with individual in the community program-in home where the client is most comfortable and assist him or her in removing barriers to healthy functioning. 

 

• We provide Psychiatric rehabilitation services as an extension to the therapeutic services an individual may be receiving currently. 


• Our duty role is to support the psycho-therapeutic aspect of an individual’s mental health treatment by unloading what transpires internally, mentally, and emotionally in individuals we serve. 


• Our rehabilitation services educate youth and adults on who they are, what their strengths and needs are, and aiding to bring resolution to psychiatric symptoms.
